To schedule an appointment, call our office at (817) 571-6863 or fill out the following form. One of our staff members will personally call you to set up a time that is convenient for you.
Your appointment is important to us. To minimize your waiting time, we ask you to arrive at least 10 minutes before your appointment so we may check you in and obtain the necessary paper work. Please bring your current insurance card to each visit and notify our staff if there have been any changes in your personal, contact or insurance information.
Your medical records are confidential. We will release these records only with your consent. Likewise, we will give no one information by telephone regarding your medical condition without your approval.
